Miss Shayla’s List – Family Support
Families navigating services for children with disabilities often need a clear starting point. The Washington State Medical Home Partnerships Project, based at the UW IHDD, is sharing a helpful resource created by parent advocate Shayla Collins.
“Miss Shayla’s List” is a brief menu of key financial, transportation, and recreation resources to support individuals with intellectual and developmental disabilities (IDD) and their families. Created by Shayla Collins, a parent and community advocate, in collaboration with Sophie Maleng, ARNP, and contributors from UW LEND, the WA INCLUDE Collaborative, the Seattle Children’s Autism Center, and other partners.
This list is designed by a parent, for parents and families and continues to grow with community input. Families and providers are encouraged to adapt this handout to their own communities and share suggestions for future updates.

About the Medical Home Partnerships Project
The Washington State Medical Home Partnerships Project for Children and Youth with Special Health Care Needs helps primary care providers, families, and community organizations work together to improve care for children and youth, particularly those with disabilities, chronic conditions, or special health care needs. Housed at the UW IHDD and funded by the Washington State Department of Health, the project promotes the medical home model: team-based, family-centered care that connects medical, behavioral, and community supports. The project’s work includes community asset mapping, family leadership development, technical assistance, and collaboration with state and local partners to strengthen inclusive and coordinated care across Washington.
